Roach, insect issues among 40 violations found inside restaurant ordered shut

Below is a list of some locations recently inspected by the Florida Department of Business and Professional Regulation.

Some have been closed for their violations and others have been warned with a “follow-up inspection required”.

It appears that locations in Broward have been closed for “fly” issues that have not been closed for the same violation in Miami-Dade County.

All locations that were ordered to close were allowed to reopen after ordered cleaning and re-inspection.

“Cockroach activity present, evidenced by live cockroaches found. Observed 4-6 or more roaches locking themselves on the floor in the third dry storage area next to the second kitchen area.”

“Live, small flying insects in the kitchen, food prep area, food storage area and/or bar area. Approximately 20 or more live flies observed on onions stored in the front dry storage area next to the ice bin machine and refrigerators standing directly in front of the kitchen area. Observed flies flying through the kitchen and landing on to-go containers where food is being prepared. Observed 2 or more flies flying around the desert area in the front counter area. Observed 2 flies flying around the uncovered steam table and dessert station and landed on pastry fondant in the dessert buffet area in the dining room.”

“Accumulation of black/green moldy substance inside ice maker/bin. Observed accumulation of black/green moldy substance inside ice machine/bin adjacent to front dry storage area.”

“Non-food grade paper/paper towel used as a liner for food containers. Observed numerous white cloths being formed as mold shapers to mold flour into a shape to be placed in a bread oven.”

“Raw animal foods not properly separated in the warming unit based on the minimum cooking temperature required. Observed raw chicken stored above raw beef within standing range in a cooler adjacent to the front dry storage area. Observed raw chicken stored over raw beef within standing range in freezer next to 3 compartment sink. Observed raw beef stored above raw goat in a standing freezer next to a 3 compartment sink.”

“Selling ban issued in time/safety food temperature control due to temperature abuse. Time/temperature control for safety food kept cold above 41 degrees Fahrenheit. Observed TCS foods kept within reach in cooler in kitchen area overnight per operator all above 41°F. cooked chicken 4 pans 46F); boiled beef, boiled goat, white rice 46F – keep cold); Cooked Rice (46F – Cold Holding) All Products 46-47°F Operator discarded all products.”

“Time/Temperature control for safety food refrigerated above 41 degrees Fahrenheit. Observed TCS food stored overnight in cooler in kitchen area per operator, all above 41°F. Kept within reach in the cooler overnight. range in the cooler; cooked chicken 4 pans 46F); boiled beef, boiled goat, white rice 46F – keep cold); Cooked Rice (46F – Cold Holding) All Products 46-47°F Operator discarded all products.”

“Time/temperature control for safety food that is reheated in a microwave oven to hold and does not reach 165 degrees Fahrenheit. Cooked chicken reheated in the microwave at 126°F-137°F has been observed to be made for catering delivery.

“Food contact surface contaminated with food particles, mold-like substances or slime. Observed can opener blade with mold-like substance. Chopping boards observed with build up of food and grease on all shelves in kitchen and crockery area.”

“Food contact surfaces are not cleaned and disinfected between working with raw pet food and ready-to-eat food or unwashed produce. Observed food contact surfaces cleaned and sanitized between preparing raw beef, reheating produce, no sanitizing solution.”

“Ready-to-eat, time/temperature control for safe foods prepared on-site and held longer than 24 hours, not properly dated. Observed cooked chicken skewers, beef skewers, beef burgers and cooked chicken all TCS cooked meats within reach in refrigerators made on 12/21. per operator in the morning without date stamp. Observed cooked lentils, cooked beef, cooked fish, cooked goat, cooked lamb and cooked carrot desert second range in the cooler, removed from the freezer, as of Monday 19th no date stamp.

About 5 live flies on the dry storage shelf above the entrance to the kitchen in the kitchen. About 10 live flies on the walls above the register and vending machine at the front counter, not in the kitchen. About 10 live flies on the walls in the dining room. Repeat violation.”
